Access Request Forms

Use the links below to download the access forms for each functional area, unless your school or division has its own form to use. Learn more about required training on the Access Roles page related to the type of access you need.

Finance

• Campus Finance Systems Access – For use by campus offices to request finance access to ConnectCarolina, Travel systems, P-Card, Surplus Property Management System(SPMS) and Customer Billing Management (CBM).
• Central Office Finance System Access – For use only by Finance & Operations, Office of Sponsored Programs, and UNC System Office to request finance access to ConnectCarolina, Travel systems, P-Card, Surplus Property Management, and Customer Billing Management (CBM).

HR

• Campus HR Systems Access – For use by campus offices to request HR access to ConnectCarolina, LawLogix, Background Check, PeopleAdmin, Reporting, and Carolina Talent.
• Central Office HR Systems Access – For use by central office to request HR access to ConnectCarolina, LawLogix, PeopleAdmin, InfoPorte, and Carolina Talent.

Student Administration – Slate, GradStar, and more

• Campus ConnectCarolina and Related Applications Access Request Tool *Student Administration* For use by campus staff to request student administration access to ConnectCarolina, Slate, GSHIP, and Gradstar.
• Central Office ConnectCarolina and Related Applications Access Request Tool *Student Administration* For use only by Undergraduate Admissions, Office of Scholarships and Student Aid, Office of University Registrar, College of Arts and Sciences Academic Advisors, Office of University Cashier (for Student Financials) and the PID Office to request student administration access to ConnectCarolina central office functions.

Data, SSN, and Reporting

• InfoPorte Access Request form Use this form to request access to InfoPorte for reporting and other applications. Please note: for HR reporting and Student reporting access, you will need to work with your ARC for access.
• TarHeel Reports Access Request Use this ticketing request form to request access to TarHeel Reports for reporting.
• Request for Access to View or Update Social Security Numbers for Student Administration and Human Resources Use this form for requesting access to view or update social security numbers in Student Administration and Human Resources.

Access Request Coordinators

• Request for Access Request Coordinator (ARC) – Related Changes Use this form to add or remove Access Request Coordinators or to assign ARC Designators (person who can sign forms to add or remove ARCs).

Unit-Specific

• System Office HR Systems Access Use this form for requesting HR access to ConnectCarolina for UNC System Office staff.
• School of Medicine access forms Use this link to find access forms specific to the School of Medicine.
