Request Medical Records from Children’s Hospital Colorado

Authorized parents and legal guardians have the right to review or request a copy of their child’s medical records.

How to request a copy of medical records

Parents and legal guardians can get a copy of a child’s medical record through MyChart or by filling out a request form. We do not accept requests by telephone.

Online

Children's Hospital Colorado now offers an online records request tool that verifies your identity by asking for a photo of your driver's license, which can be taken via webcam or smartphone. This tool is available in both English and Spanish. There is no additional charge to use this service.

Patients

Healthcare providers (Doctors, nurses, therapists, etc.)

MyChart

You can request medical records through MyChart. These requests will take three business days from date of receipt to process. If you have questions about MyChart, call 720-777-6939.

You can also get a copy of your child’s medical record by filling out a request form. Processing medical record requests will take up to 10 business days from date of receipt.

Print, complete and sign the form below. Return the form to us by mail or fax. The address and fax number are located at the bottom of the authorization form. You may also email the form to roi@childrenscolorado.org or leave it with the Release of Information Department during business hours.

We will release and deliver medical records to you in the method you select on the authorization form. We can release and deliver records to you through mail, email or fax.

Note: The Release of Information Department cannot review, discuss or interpret medical records. Please direct these questions to your healthcare provider.

Non-patient/non-family requests for medical records

The following forms are for non-patient/non-family third party requesters. Please fill it out, sign and return by mail or fax. The address and fax number are located at the bottom of the authorization form.

Important notes about your child’s medical record request

We recommend requesting a minimum set of records. Requests for complete records (including progress notes, consent forms, flow sheets, etc.) can delay processing. It can also become very costly and usually contains much more information than is needed. For guidance on how to decide what to request, please contact the Release of Information Department.

Personal representatives will not be provided access to a minor’s records if the minor was legally able to consent for treatment, unless the minor authorizes access to this information.

We may deny access if the information is not part of the designated record set or if we determine the access could endanger or harm the patient or others.

We require patient signature if the patient is 13 years of age or older and you are requesting treatment information for reproductive health (including pregnancy and sexually transmitted disease), HIV/AIDS or drug/alcohol.

We require patient signature if the patient is 12 years of age or older and you are requesting behavioral health or psychiatric care information.

Charges for medical records

Colorado State Law authorizes Children’s Colorado to charge for copying medical records. Charges, if any, are due at the time records are received. The Release of Information Department accepts check and credit card payments only. You can also make an online payment if you have a medical record invoice.
